Celldex's Barzolvolimab Hits Both Phase 3 Hives Trials; Anaphylaxis Cases Clip the Stock

Celldex Therapeutics (CLDX) reported Sept 22 that barzolvolimab, its anti-KIT antibody, hit the primary endpoint and all key secondary endpoints in both Phase 3 EMBARQ-CSU1 and EMBARQ-CSU2 trials in chronic spontaneous urticaria (CSU). The efficacy is the strongest ever reported in the disease. The stock still fell roughly 12% as investors zeroed in on two anaphylaxis cases in one treatment arm.

The efficacy numbers leave little to argue with. At 12 weeks, complete response, the total absence of itch and hives measured as a UAS7 score of zero, reached 42% to 46% across the two barzolvolimab doses versus 9% to 13% for placebo. By week 24, complete response climbed as high as 54% in one trial against 15% to 18% for placebo. The primary endpoint, mean change in UAS7 at week 12, showed reductions of about 20 points versus 11 for placebo, all with p-values below .00001.

The differentiation Celldex is selling sits in the omalizumab-refractory subgroup, the patients Xolair does not help. There, the 150 mg dose drove complete response of 55.3% in EMBARQ-CSU1 and 41.7% in EMBARQ-CSU2, versus 9.3% and 15.1% for placebo. Barzolvolimab depletes mast cells by blocking KIT, a different axis from Xolair’s IgE blockade, which is why it works where omalizumab fails.

Safety is why the stock sold off. Two anaphylaxis cases occurred in one barzolvolimab-dosed group, alongside one case in the placebo group, a signal investors read as a potential boxed-warning risk. CEO Anthony Marucci called the incidence “extremely low” across roughly 2,400 treated patients, and Stifel’s Alex Thompson noted Dupixent already carries an anaphylaxis warning and still posts blockbuster sales.

The trials continue to 52 weeks, and Celldex plans a 2027 BLA submission. Cantor Fitzgerald’s Kristen Kluska called it “the best Phase 3 data set we’ve seen” in CSU and forecast $4.8 billion in peak sales.

The readout landed as the thesis-defining event for a company whose earlier barzolvolimab readout missed in prurigo nodularis. Celldex traded around $38 before the news, closed at $31.12 on Sept 23, and was near $31.48 on Sept 24. On efficacy, barzolvolimab delivered. The market is now pricing the safety tail.

What It Means for the Trade

The 12% selloff on the best efficacy dataset ever reported in CSU is a lesson in how binary markets treat safety surprises: 42-46% complete response at week 12 against 9-13% placebo is a blowout, but two anaphylaxis cases in one dosing arm put a potential boxed warning on the table, and boxed warnings change launches. The actual risk calculus is murkier than the tape implies: one anaphylaxis case in placebo against two in one barzolvolimab group is not a clean causal read, Dupixent carries an anaphylaxis warning and still posts blockbuster sales, and roughly 2,400 treated patients give the denominator Marucci pointed to. The trade here runs on a 2027 BLA submission, meaning no near-term regulatory catalyst — between now and filing, the stock trades on safety interpretation and the ongoing 52-week data. Cantor’s $4.8 billion peak-sales estimate prices the best case: Xolair-refractory dominance plus a share of biologic-naive patients. Position sizing should reflect that a boxed warning or a monitoring protocol could shave launch velocity materially.

Mechanism and Market Context

Barzolvolimab depletes mast cells by blocking KIT, the receptor for stem cell factor that mast cells need to survive. That is upstream of essentially all mast-cell-mediated disease — histamine, IgE, and cytokine release collapse when the cell population does — which is why a KIT blocker can work where omalizumab, which neutralizes IgE downstream of the mast cell, fails. The 55.3% and 41.7% complete response rates in the omalizumab-refractory subgroups are the numbers that define the commercial opportunity, because refractory patients have no good option today. The same biology cuts both ways: KIT is expressed in other lineages, so off-target effects on those populations are where the safety tail risk lives. The prurigo nodularis miss showed the mechanism is not universal across conditions; CSU is where mast-cell dependence is cleanest.

Risks and What Could Go Wrong

  1. A boxed warning or monitoring requirement for anaphylaxis is the main regulatory swing factor ahead of the 2027 BLA.
  2. The 52-week extension could surface late-emerging safety signals a 12-week snapshot misses.
  3. Other KIT-directed programs and next-generation biologics could compress the refractory-niche window before launch.
  4. With the stock off roughly 12% from pre-readout levels, negative sentiment can persist on interpretation alone; there is no data catalyst until the longer-term readout.
